

The election commission of Punjab has started Special Investigative Revision (SIR) across the state. Booth level officers (BLO) have been appointed to fill enumeration forms for the people. The officers reported that the public is facing difficulties in filling out forms. To curb the situation, Officials started going to houses of individuals to help them. BLO Jaswinder Singh stated that enumeration forms are very easy to fill. These have to be filled in three parts.
Parts of the form
The enumeration forms are divided into 3 main parts. The first part of the form i.e. the left side has to be filled by users that have been registered under 2003 voter list or are above the age of 40. While people of age 40 or below will fill the right side of the form. However, the third part of the form is to be filled by everyone irrespective of the age.
Online form
The election commission has also put out online forms for the public. This can be filled on the official website of the Election Commission under the Enumeration column. To fill the form, name, phone number and EPIC number has to be registered. After submitting the necessary details, the enumeration form can be filled.
Steps to fill the form
Step 1: Top Center column
The first part of the form includes name and number of the area’s BLO along with preliminary details of the elderly of the house. For security reasons, a QR code along with the photo is printed. This column is not for individuals to fill.
Step 2: Left column
This is the first important part of the form. This column is for those who have registered names under the 2003 voting list or are of age above 40. Few important details that are to be filled in this column are Name, Voter card number, Name of the father/ husband with relation in the adjacent box. Some preliminary details like Age, Gender and Assembly Constituency have to be mentioned. The details of old Assembly number, Part number and Serial number have to be mentioned in the small boxes given below.
Step 4: Right column
This is the second most important part of the form. This has to be filled by people who are not registered under the 2003-2004 voter list. For example, a newly wed woman, a child who just turned 18 or anybody who is not registered in the last voter list. In any of these conditions, the preliminary details of the member have to filled along with the Assembly constituency.
For people who do not have a voter card, Not Applicable has to be written under Voter card number. Part number and serial number of the elderly has to be written to automatically link it with the family. If no such conditions apply, this column has to be left untouched.
Step 5: Declaration and Signature
A legal declaration is written at the bottom of the form that is to be filled after filling all the necessary details. Just under the legal declaration a part of “Signature of the voter or any adult” is mentioned. Here a thumbprint or Signature is valid along with the date.
Steps for Online form submission
Step1: Visit the official website or app
To fill the form the users initially can either visit voters.eci.gov.in or ceopunjab.nic.in on the browser. For phone users Voter Helpline App is available of Google play store and Apple app store.
Step 2: Login or Register
For old users, logging in using phone number and OTP is required. On the other hand, for new users, Sign up using mobile number and Email Id and create a new account.
Step 3: Click on SIR link
After successfully signing in, click on the "Special Intensive Revision (SIR) - Enumeration Form" or "S.I.R. Entry Portal" link on the home page. For this EPIC number has to filled. Thereafter, a Digital Enumeration form will open
Step 4: Details of Existing Family Member
The foremost thing on the portal is to select the State, District, and Assembly Constituency. If you are already registered under the voter list, the left side of the screen will show all the details and a verification of these details is required.
Step 5: Data entry of the new member
If a new family member has to be added in the voter list, or who is not registered under the 2003 voter list, click on "Add Family Member" or "New Enumeration Entry". Here preliminary details of the new member and the details of the elder member of the family has to be typed.
Step 6: Provide your Aadhaar number, mobile number, and family details.
The third part of the form starts from this section. Here the 12 digit Aadhaar number of the user has to be filled along with the operating mobile number. After this, Names of parents and spouse is to be filled along with their Voter Id Card number.
Step 7: Upload and submit
The biggest advantage of the online format is that you don’t have to stress around with papers in your hand. Online submission of the form in Pdf format is required.
After submission of the form, a receipt or reference number will pop on the screen. You can either note the number or take a screenshot of the same and this will be transferred to the BLO’s app. The BLO will physically verify the form by visiting your residence and mapping your vote online.
